Dementia navigators – Dementia post-diagnostic support service
The post-diagnostic support service is an NHS funded service delivered by Alzheimer’s Society (www.alzheimers.org.uk) Dementia Advisers, and commissioned by Lancashire and South Cumbria Integrated Care Board. Alzheimer’s Society is partnered with Age UK Lancashire for further services, Dementia Hubs and Cognitive Stimulation Therapy, connecting patients to two wide expert support avenues.
The service provides expert post-diagnostic dementia problem-solving, information, and advice available for patients and/or carers. The service is available for patients and carers living in Central, West, East and North Lancashire, Blackpool, and South Cumbria. Referrals to the service are for those individuals who have received a dementia diagnosis or are presenting with increased dementia symptoms identified in a consultation or annual dementia review, which could benefit from strategies, understanding, or system navigation. Alzheimer’s Society has over a decade of local experience in delivering such services to patients and their carers.
Eligibility
The service is available for all adults, diagnosed with any form of dementia or carers of people diagnosed with dementia
Services provided by the Dementia Navigator Services’ Dementia Advisers:
- Practical problem solving, strategies and communication ideas
- Information and advice
- System and local services navigation
- Available as and when new symptoms, challenges, needs arise
- Range of group support including carers information courses, and connections and signposting to local activities
- Alzheimer’s Society national services, including Dementia Support Line (including evening and weekends), web forum Dementia Support Forum, Companion Calls, and extensive advice and learning factsheets and publications are also available from www.alzheimers.org.uk
